What Christopher says is exactly how I dealt with it with the Padawans.  I had a couple who said they would not fill it out because they did not want to put the ssn on the FRN form. Once I explained that they are doing it themselves so that no one else has access to the ssn, and that it is just going from one gov site/agency to another, they were okay with it. One parent came in on test day and wanted to do it in front of me, he did, no issues.
