DXCC Items Order Form

After all of the chat about 'extra' items such as certificates, I have a question about the Order Form.

On the Help page it says to Mail, Email, or Fax the form to ARRL.
I know how to Fax, and (snail) Mail, but I cannot edit the PDF to Email the form.
How can I Email this form in order to speed processing?
(I am reluctant to Fax my CC info...)

I downloaded the .pdf, the did a Google search and found a web site that can convert a .pdf to a Word document.  I edited the Word doc and then saved it as a .pdf and emailed it.  Worked, quickly.
